Bugs about H.264 of VA-API and Microsoft Media Foundation

Voukoder reported an error when I was rendering with VA-API H.264:

When I was rendering with Microsoft Media Foundation H.264, the rendered video contains glitch:

VA-API requires uploading the frames to the encoding device first (and one HW encoding device needs to be present of course).

I need to fix the media foundation issues.
